Vendor SOP & Item Declaration Form

All vendors must complete this form before load-in and list all items being brought into the event space. Events over 250 guests may require an additional COI.

Vendor / Event Information

Standard Operating Procedure

  1. Vendor must check in with event staff upon arrival before unloading any items.
  2. Vendor must complete the Vendor / Event Information section in full.
  3. Vendor must list all items, equipment, products, materials, decor, tools, boxes, carts, or other property being brought into the venue/event area.
  4. Any restricted, hazardous, or specialty items must be disclosed to event staff prior to entry.
  5. Event staff may review the item list during load-in and may request clarification or removal of unapproved items.
  6. Vendor is responsible for removing all listed items at the end of the event unless otherwise approved in writing by event staff.

Items Being Brought In

Vendor must list every item being brought into the event area.

Items Being Requested

Please list any requests from the venue — for example: 6ft tables with black spandex, power. The venue does not guarantee it can fulfill requests.

COI Requirements

COI calling out AREA15 and its additional insured 1MM coverage for General Liability, Auto Liability, and Workers Comp.

Additional insureds: Area 15 Las Vegas LLC, Integral Park Place Center-Las Vegas LLC, Paradise Las Vegas LLC, New Vegas Commercial Acquisition LLC, New Vegas Holding Company LLC and Cushman & Wakefield of Nevada.
